A: The Licensure Data Download provides the practitioner name, license information, mailing address, and email address for health care practitioners and establishments licensed in Florida. Click here for the Licensure Data Download webpage.


Q: I am seeking detailed information about medical doctors, such as disciplinary actions taken against MDs since 2010. Where can I find this information?

A: The Practitioner Profile Data Download provides the detailed information about practitioners, including disciplinary actions, faculty appointments, certifications, staff privileges, and a host of other information. The data is submitted by the practitioner and has not been verified by the Department unless otherwise indicated. The physicians file the data with the Division of Medical Quality Assurance as required by law. The 5 professions required to provide this data are Medical Doctors, Chiropractors, Osteopathic Physicians, Podiatric Physicians, and Advanced Registered Nurse Practitioners. A: The Licensure Data and Practitioner Profile data are provided in text delimited format. You must first save the data as a text file (.txt), and then import it into a spreadsheet program like Excel or a database program. Q: I get an error when trying to download files from the Licensure Data File Download Page. What can I do?



A: The Department of Health's download site is secure and requires what is called a "reverse DNS lookup" before allowing a connection. Most errors with downloading from the Licensure Data File Download Page are because the download site cannot do this. This is an issue that needs to be addressed by your office or internet service provider's (e.g. Comcast, Sprint, AOL) technical support staff. Please check with them to verify your DNS settings. Inform them that you are trying to download files from an ftp site that requires reverse DNS lookup of your host.
